Our hospice social workers are here to listen, guide, and advocate for you. They can assist with:
Emotional Support: Helping residents and families cope with feelings of grief, stress, and uncertainty. Offering guidance and mediation for families and loved ones facing difficult conversations and decisions.
Advance Care Planning: Assisting with decisions about care preferences, legal documents, and future planning.
Community Resources & Referrals: Connecting you with caregiver support services and other helpful resources within the hospice and the community.
Legacy Projects: Facilitating meaningful keepsakes such as hand molds and memory projects to honour loved ones.

Bereavement Support is Available to Everyone
Grief is a deeply personal journey, and no one should have to walk it alone. Rotary Hospice Stratford Perth offers a range of free bereavement programs to community members who have experienced the death of loved one, whether at Rotary Hospice, or elsewhere.
These programs are created and facilitated by our Rotary Hospice Registered Social Workers, who have specialized education in grief and bereavement.

Steps for Support
This is a weekly walking group, where participants receive peer support from others who are grieving in the community. The walk is led by volunteers who have received hospice bereavement training. The walk takes place every Tuesday morning at 9:30am in Stratford. There is a short intake to complete prior to joining.
Group Support
Our social workers facilitate quarterly bereavement groups throughout the year, both virtually and in-person. Focusing on various losses, these groups will include time to share your grief experience, learn coping strategies, and allow for the opportunity to make connections with others who are grieving.
Presentations & Webinars
Our quarterly webinar series is offered virtually over Zoom in March, June, September, and December. Hosted by our social workers, these sessions include a presentation and a Q&A. Additionally, join us at our community educational workshops that cover a variety of end-of-life and grief topics.

We believe in the importance of remembrance and coming together as a community to celebrate the lives of those we have lost.
Lights of Love: A heartfelt gathering in early winter, where families and friends gather to honour and remember their loved ones.
